INTERGRATED TEAM CARE - ITC
The Integrated Team Care (ITC) program aims to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic conditions through care coordination and support for self-management.
Katungul's ITC can assist you to book your appointment, complete any forms, and liaise with your GP.
We can also help you access:
Assistance to attend their referred medical specialists appointments
Assisting with transport costs
Support with the purchase of medical aids as suggested by specialist
ELIGIBILITY CRITERIA
To access the ITC program, you must be a Katungul client with a current GP Management Plan, GP referral and have a chronic health condition.
All clients will be required to complete registration paperwork. Clients can access our ITC program across all three clinics.

Katungul Aboriginal Corporation Regional Health and Community Services currently has three clinics that offer medical, dental and specialist services; one in Narooma, Bega and Batemans Bay. Outreach programs and specialist programs are still in place to aid the Koori people.
