INTERGRATED TEAM CARE - ITC


The Integrated Team Care (ITC) program aims to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic conditions through care coordination and support for self-management.

Katungul's ITC can assist you to book your appointment, complete any forms, and liaise with your GP.

We can also help you access:

  • Assistance to attend their referred medical specialists appointments
  • Assisting with transport costs
  • Support with the purchase of medical aids as suggested by specialist

ELIGIBILITY CRITERIA


To access the ITC program, you must be a Katungul client with a current GP Management Plan, GP referral and have a chronic health condition. 
All clients will be required to complete registration paperwork. Clients can access our ITC program across all three clinics.
